ASSISTANT MANAGER of CARE COORDINATION

Description: Responsible for integrating, facilitating, and coordinating patient care for high risk patients across the healthcare continuum.  Ensures the delivery of cost effective, outcome orientated and quality care within an appropriate LOS.  Manages patient care to prevent fragmentation and duplication of services.  Works in a collaborative practice with the physician, charge nurse, payors, and other members of the healthcare community to meet patient specific needs.

Requires: BS degree or higher as a Social Worker or RN; five (5) years exp as an RN Case Manager or Social Worker.

Prefers: CCM certification; advanced computer literacy and data analysis skills; and two (2) years of supervisory exp.

SURGICAL TECH / CERTIFIED

Description:  To perform a variety of duties as a member of the surgical team during the patient’s surgical intervention.  Assists with the preparation of surgical patients, creates/maintains sterile field, issue instruments/equipment to surgeons and assistants.  Work is performed under the guidance of a registered nurse/surgeon in accordance with established hospital standards, established regulatory and accreditation requirements and objectives of the hospital and OR/RR dept.

Requires: HSD/GED, NYS Surgical Technologist Certification, BLS, and must be within a 10 minute radius of hospital for OR call.

Prefers: One year of previous OR experience.

Description:  To provide nutritional care to patients in the form of nutrition assessment, patient and family education, computerized medical record documentation, and related clinical responsibilities.

Assists in scheduling tests.

Description:To implement the Nursing Facility’s Occupation Therapy programs and services under the guidance of a Licensed Occupational Therapist, in accordance with policies and procedures and within the scope of practice of a COTA.

Requires: Certification as an Occupational Therapist Assistant by the National Board of Certification of Occupational Therapists. Applicants must possess a working knowledge of NYS and federal codes pertaining to the age of the residents and patients served, knowledge of principles of growth and development over the life span.
