Maintenance Mechanic IIJOB TITLE: MAINTENANCE MECHANIC II DEPARTMENT: EngineeringDESCRIPTION: The purpose of this job is to maintain all internal/external structural & mechanical integrity of all Hospital/Nursing Home properties and equipment, and to perform facility specific projects and renovations. The job specifically is intended to attract individuals who have gained a level of mastery in particular trades or technology, essential to the organization meeting its mission.EDUCATION: Possess a high school diploma or equivalent education.LICENSURE/CERTIFICATION/REGISTRATION: To achieve goals of the department, possess a level of mastery in one of the following: carpentry, plumbing, electrical or steel-construction. Proficiency and licensure to handle HVAC building systems, pesticides and/or a two-year degree in building system automation preferred. EXPERIENCE: Possess 5-years of experience in commercial, institutional, industrial, or military plant maintenance. Demonstrates mechanical and electrical ability.PREFERRED: Possess experience in healthcare plant maintenance, with an established understanding of NFPA Life Safety Code 101, NYS-DOH, and DNV requirements for healthcare facilities. ABILITIES: Ability to perform the essential functions of the job. Ability to operate and understand power tools, job specific computer operations, electrical meters, electrical switches and circuit panels. Ability to read blueprints and schematic drawings. Ability to perform basic computational skill sin order to troubleshoot and estimate material requirements. Primary/Specialty Care Case ManagerThe purpose of the Case Manager is to provide care management services for high-risk patients in the Primary/Specialty care population and work with behavioral health agencies outside the Nathan Littauer Hospital Primary/Specialty Care system to coordinate PCP services for the agency’s high-risk population.EDUCATION: Possess a High School diploma or equivalent. Graduate from an accredited Licensed Practical Nurse program. PREFERRED: Possess an Associates’ Degree with a focus on Care Management or related disciplinePossess Improvement; Quality Improvement and Patient Centered Medical Home recognition is highly preferred.LICENSURE/CERTIFICATION/REGISTRATION: Possess current NYS license for Licensed Practical Nurse.EXPERIENCE: Possess prior Care Management experience.SHIFT: 8:00 AM – 4:30 PM STATUS: 40 HOURS
