Emergency Room Community NavigatorDescription: As a member of the Care Coordination team, the Emergency Room Community Navigator is responsible for providing a full range of medical, social and case management services to patients by providing an appropriate level of evaluation to determine/identify bio-psych-social characteristics and social determinates of health needs of the patient and provides appropriate interventions for patients who are discharged from the Emergency Room as well as those patients admitted to the facility. Assist with patient/family and other hospital disciplines access to necessary services that extend the continuum of care beyond the hospital stay.Requires: Possess a Bachelor’s degree or higher in Human Services or related field.
