Description:  The purpose of this job is to evaluate patients, develop care plans and administer or direct the administration of physical agents and procedures utilized in rehabilitative medicine as prescribed by a referring physician, taking into account the individual patient’s special physical or age-related needs.

  • Possess a Bachelors of Science Degree in Physical Therapy.
  • Prefer Masters of Science Degree in Physical Therapy.
  • Possess a New York State Physical Therapist License.  Possess CSCS (Certified Strength & Conditioning Specialist) certification.
  • Possess no prior experience.  New graduate is acceptable, on the job training will be provided to new graduates for up to six (6) months.
  • Prefer prior experience in a rehabilitative medicine setting.

Status:  7:30am – 4:00pm (40 hours)

Physical Therapist

Description:  To perform skilled therapy interventions for a geriatric and rehab population.

Requires:

  • HSD/GED
  • Bachelor’s degree in Physical Therapy
  • Prefer Master of Science Degree in Physical Therapy.
  • Possess York State Physical Therapist license.
  • Prefer one (1) year prior experience
  • Prefer prior Meditech scheduling experience

Status:   7:30am and 4:00pm (Casual) work 6-8 hours on Saturday or Sunday

From:  $32.56 to $48.85 per hour.  

Physical Therapist

Social Worker

Description:  The purpose of this position is to provide a social service program that meets the psychosocial needs of the individual resident which will provide services, based upon a comprehensive assessment, which will assure the maximum attainable quality of life for the residents, the residents emotional and physical well being, self determination, self respect and dignity taking into account the individual resident’s special physical or age-related needs.  The population served includes adult, geriatric residents.

  • Possess an Associate’s of Arts degree in Humanities and Social Services.
  • Prefer Baccalaureate degree from an accredited college or university in social work or gerontology.  Holds a Master’s degree in social work or is a Certified Social Worker and has pertinent experience in a health care setting.
  • Possess a NYS drivers license.  Certified NYS Screener or obtains certificate within the first two months of employment.
  • Possess experience in long term care and/or healthcare.
  • Prefer experience in geriatric long term care.  Experience in areas such as Trauma Informed Care and Substance Abuse Disorder treatment preferred.  Experience with discharge planning process preferred.

Status:  8:00am – 4:30pm (40 hours)

From:  $25.14 to $36.45 per hour.  

Director of Recreation Therapy

Description: The purpose of this job is to provide a strong well-organized recreation program to meet the social, emotional, physical and cognitive needs of the residents seven (7) days a week taking into account the individual resident’s special physical or age-related needs. To develop programming to stimulate individual interests in meaningful activities.  To provide a therapeutic environment for all residents. 

  • Possess a Baccalaureate degree from an accredited college or university with a major in therapeutic recreation or a major in recreation with an option in therapeutic recreation.
  • Prefer Master’s Degree from an accredited college or university with a major in therapeutic recreation or a major in recreation with an option in therapeutic recreation.
  • Certification as a Recreation Therapist.  Clearance through the New York State Control Register or Child Abuse and Maltreatment.
  • Prefer three (3) years experience as a Recreational Therapist.
  • Possess a working knowledge of Federal and State Medicare and Medicaid conditions of participation; New York State Codes, Rules and Regulations.

Status:  8:00am – 4:30pm (40 hours)  Hours will fluctuate depending on departmental needs.
