Description:  A Unit Assistant performs direct nursing duties related to the personal care and comfort of the residents.  Assists the Certified Nursing Assistant in the administration of resident care.  Performs services necessary for the cleanliness and safe functioning of the Nursing Unit.

  • Possess a high school diploma or equivalent.
  • Demonstrate competency with a Competency Skills Evaluation tool once trained.
  • Continued employment is contingent on obtaining certification as a Certified Nurse Assistant in New York State within the specified timeframe.
  • Prefer prior experience in a Skilled Nursing Facility.

Status:   10:45pm – 7:15am (40 hours)

Social Worker

Description:  The purpose of this position is to provide a social service program that meets the psychosocial needs of the individual resident which will provide services, based upon a comprehensive assessment, which will assure the maximum attainable quality of life for the residents, the residents emotional and physical well being, self determination, self respect and dignity taking into account the individual resident’s special physical or age-related needs.  The population served includes adult, geriatric residents.

  • Possess an Associate’s of Arts degree in Humanities and Social Services.
  • Prefer Baccalaureate degree from an accredited college or university in social work or gerontology.  Holds a Master’s degree in social work or is a Certified Social Worker and has pertinent experience in a health care setting.
  • Possess a NYS drivers license.  Certified NYS Screener or obtains certificate within the first two months of employment.
  • Possess experience in long term care and/or healthcare.
  • Prefer experience in geriatric long term care.  Experience in areas such as Trauma Informed Care and Substance Abuse Disorder treatment preferred.  Experience with discharge planning process preferred.

Status:  8:00am – 4:30pm (40 hours)

From:  $25.14 to $36.45 per hour.  

Physical Therapy Assistant

Description:  The purpose of this job is to provide technical physical therapy services in accordance with patient care plans, department policies and procedures and within the scope of practice for Physical Therapy Assistants taking into account the individual patient’s special physical or age-related needs.

  • Possess high school diploma or equivalent.
  • Possess associate degree in physical therapy.
  • Possess a New York State license as a Physical Therapy Assistant.

Status:  7:30am – 4:00pm (40 hours)

From:  $21.34 to $25.88 per hour.

Administrative Assistant to the DON Nursing Facility

Description:  The purpose of this job is to support the Director of Nursing and the Nursing Department in the Nursing Facility, with secretarial, administrative and operational duties related to nursing.

  • Possess a high school diploma or equivalent.
  • Prefer Associates degree in applicable field.
  • Possess three (3) years experience in Nursing Home or related field.
  • Prefer computer literate, with working knowledge of Microsoft Office.  Excellent verbal and written communication skills.

Status:  8:00am – 4:30pm (40 hours)

From:  $18.95 to $21.95 per hour.

Director of Social Work

Description:  The purpose of this position is to provide a Social Service Program that meets the psycho-social needs of the individual resident which will provide services, based upon a comprehensive assessment, which will assure the maximum attainable quality of life for the residents, the residents emotional and physical we–being, self-determination, self-respect and dignity, taking into account the individual resident’s special physical or age-related needs.  The population served includes adult, geriatric residents.  Under the supervision of the Administrator, the Director of Social Work is responsible for the supervision of social work services within the facility. 

  • Possess a high school diploma or equivalent.
  • Possess an Associate’s of Arts Degree in Humanities & Social Services.
  • Prefer Baccalaureate Degree from an accredited college or University in Social Work or Gerontology.  Holds a Master’s Degree in social work or is a Certified Social Worker and has pertinent experience in a healthcare setting; experience with geriatric population preferred.
  • Possess a New York State Drivers License.  Certified NYS Screener or obtains certificate within the first month of employment.
  • Possess experience in long-term care and/or healthcare.
  • Prefer experience in geriatric long-term care.
