The Specialty Surgery Site Manager has responsibility and accountability for all patient care activities, site operations, staff function and physical space integrity.EDUCATION: Graduate of an accredited school of nursing. Possess a current New York State License as a Licensed Practical Nurse or Registered Nurse. PREFERRED: Possess a Bachelor of Science Nursing degree. Primary/Specialty Care ManagerTITLE: Primary / Specialty Care Manager SHIFT: 8:00AM – 4:30PM STATUS: 40 HOURSThe purpose of this job is to identify and provide care management services for the high-risk patients in the Primary/Specialty Care population and work with behavioral health agencies outside the Nathan Littauer Hospital Primary/Specialty Care system to coordinate PCP services for the Agency’s high-risk population.EDUCATION: Possess a High School Diploma or equivalent.PREFERRED: Associate’s degree with a concentration on care management or related discipline.EXPERIENCE: Care Management experience.
