SIGN-ON BONUS FOR FULL-TIME FAMILY HEALTH NURSE PRACTITIONER

We are seeking an experienced Family Practice NP for our Center in Amsterdam, NY.  The Center consists of one (1) Internal Medicine and three (3) Urgent Care providers.  The PCP Practice is robust, working through a waiting list of patients from the retirement of community physicians and a national, statewide and regional shortage of Primary Care Providers. 

The Center has been in operation since 2021, providing services to all surrounding communities.  This will be the second PCP practice we will open there. 

We are currently in need of an Nurse Practitioner comfortable with Adult Medicine and experienced with CAD, COPD, CHF and DM.  The Nurse Practitioner should also be conversant with the principles of Chronic Care Management and serve as a resource for our CCM program while caring for this patient population.  

This requirement does not preclude the Nurse Practitioner from seeing patients of all ages. 

We are open 5 days a week.  We are seeking a FT Nurse Practitioner to attend 4, 4.5 or 5 days a week.  

We deliver preventative care, Urgent care, Chronic Care Management, school physicals, Immunizations, X-Ray, CT scan, DOT exams, blood draw and limited lab on site. 

The provider is supported by other Primary and Specialty Care providers in the Littauer network.
