MDS CoordinatorDescription: The MDS Coordinator Nurse is a LPN or RN who works under the direction of the Director of Nursing. Responsible for the input of information into the MDS, coordination of all MDS assessments, CAAS and Care Plans under the supervision of the Consultant RN MDS Nurse. Work is performed in accordance with established Nursing Home standards, Nursing department structure, established regulatory requirements and consistent with the philosophy, goals and objectives of the Nursing Facility and Nursing department. The MDS coordinator will coordinate and lead care conferences.Possess a high school diploma or equivalent.Possess an AAS degree from an accredited school of nursing.Prefer Bachelors of Science Degree in Nursing.
